Since prostaglandin H synthase is inhibited by NSAIDs production of prostaglandins cannot occur, which results in vasoconstriction and causes decreased blood flow resulting in ischemic necrosis. Due to the presence of high concentrations of toxins in the papillae and the ability of the papillary cells to produce reactive metabolites, the reactive metabolites produced cause oxidative stress. Oxidative stress causes the papillary cell injury and results in renal papillary necrosis.
